Consent Form

TO WHOM IT MAY CONCERN

I hereby consent that FEP SEARCH GROUP, A Division of Future Executive Personnel Limited has the right to collect and use or disclose my personal information for the purpose of assisting me in finding a suitable employment opportunity.

I further consent to the disclosure of such personal information to:

1. FEP Search Group’s clients when, at FEP Search Group’s exclusive discretion, the client may provide me with suitable employment.

2. FEP Search Group’s representatives (including affiliates, partners, etc.) who assist FEP Search Group in providing with full staffing services.

I consent that I will provide FEP Search Group with an updated resume as and when required.

I further consent to the retention of my personal information by FEP Search Group in their files.

I agree that FEP Search Group can share my personal information for background, educational, credit and reference checks, with out-sourced payroll service provider, Employee testing centers and with government bodies & law enforcement agencies, if required.

I consent that my personal information can be disclosed for FEP Search Group’s business purposes or as required to meet legal and regulatory requirements.

In the event that I wish to withdraw my consent to the use, disclosure and retention of my personal information, I understand that it is my responsibility to inform my respective FEP consultant and Privacy Officer at FEP Search Group in writing.

I confirm that I have reviewed the FEP Search Group Privacy Policy and acknowledge and agree that my Personal Information will be collected, used and disclosed by FEP Search Group in a manner consistent with the FEP Search Group Privacy Policy as such is amended from time to time.

I fully understand the above consent statements and am entering into them voluntarily as certified by my acceptance below.
